根據Professor Gracovetsky提出的脊椎引擎理論(Spinal engine theory) ，行走其實是是一種能量的傳導作用，在正常的活化核心肌群與呼吸模式下，脊椎、骨盤與下肢才可以維持在理想的排列，讓步態中能量的消耗（如動能、位能與彈性位能等）達到最小，行走的效率達到最高。倘若下肢的排列出現問題，便會造成不順暢的動力鏈傳遞，使得步態週期中出現代償作用，長期下來容易導致足部、膝蓋、髖、骨盆甚至是脊椎受力不均勻，進而造成疼痛。臨床上很多個案或客戶都會抱怨，在日常生活中沒有受到明顯的創傷，但下肢的各個關節有不明原因的酸痛 ，甚至連骨盤和下背部都有影響，進而影響到整個步態週期，這些都有可能是由於日常生活中使用代償性的呼吸模式而所產生低效率步態所導致。
